To receive a voucher, just send your details (name and address) along with a cheque for £14.99 made payable to MediPAL. We will by return of post, send you a MediPAL^® voucher and an application form in a presentation envelope, ready for you to give it as a gift. The person receiving it completes the form and returns it to us with the voucher; we will then issue their MediPAL^® card to them. Easy.
