When you purchase a MediPAL® card from us, we offer the facility to update the text on the card, should your medical or contact details change. Each new purchase includes two free updates for the life of the card.
To update your MediPAL card:
You can use the form we sent you with your original purchase or previous update.
You can open the attached file below, PRINT the form, complete and post back to us at the address below.
You can also save the form to your computer and email back the completed form as an attachment.
Also, scan your completed form and email back the scaned document as an attachment.
